Many clients who seek to have the FUE type of donor harvested transplant are already wearing their hair very short. For them, the matter of needing to “buzz cut” the hair on the donor area is of little concern, because it takes only days for it to grow back to a suitable length to blend in.

However, the issue of the donor area shaving becomes a practical limitation for those who don’t normally wear their hair short , and where they prefer not to make such a noticeable change in order to have an FUE transplant.

As a result, here is something I hear a lot:

I want an FUE procedure, but the idea of having to cut my hair very short makes it impractical. Do I have to use the strip method, or go someplace that advertises FUE without shaving?

No! You can still do a precision, robotic ,FUE; free of any worries about shaving the donor area!

We have developed a simple and very effective way to deal with the issue of needing to shave the donor area for extraction of the follicles by the FUE method. It’s a perfect fit with the precision, laser guided, ARTAS® FUE procedure.

You can leave the procedure with no outwards evidence that anything was done in your donor area, and the high quality of a precision laser guided FUE will not be compromised by attempting to extract without shaving; using far less advanced hand held methods that are NOT precision, laser guided methods; such as the ARTAS® is.

We call this the Conceal System® a post- FUE concealment system. As the name implies, it utilizes a temporary hair system that is pre-planned and custom made in advance of the procedure. It will match your hairs natural color, texture and length.

Immediately after the procedure, the system piece is precisely fitted into the shaved donor area, where it blends in perfectly with your existing hair. In this manner, there is no requirement for cutting your hair or drastically changing your hair style to have an ARTAS® FUE transplant at The Holt Hair Restoration Center. You will simply use the system until the donor area hair has grown back to the desired length. We can even have the system ready to go on the day of the procedure, and can place it before you leave the office! In other words, you can leave the procedure looking no different in the back than when you walked in!

It uses a breathable, mesh backing, and a medical grade adhesive tape strip to hold it firmly in place. You can actually leave it on to shower and to cleanse the donor harvested area right away without removing it! You can swim, bike, run, and exercise; all of your usual activities; with no worries about the appearance during healing and regrowth.

It can also be easily removed with rubbing alcohol, and then reapplied as needed or desired. The heat sensitive tape will re-adhere easily with body heat, or faster using a hair dryer. The tape can be easily replaced if needed.

Typically it takes no more than a few weeks for your hair to grow back to a length needed to discontinue the system. It can be saved for future procedures should you have any.

Follicular Unit Extraction

An Update on the State of the Art

Most of you, who have researched hair transplantation surgery in the past few years, would likely have come across the term FUE. This is the abbreviation for the donor harvesting technique called Follicular Unit Extraction. We compare this technique to the more conventional and more commonly performed “strip harvesting” method.

These are two very different approaches to donor hair harvesting from the back and sides of the scalp. It is only this harvesting part of the transplantation that differs between FUE and the strip harvesting method. The actual grafting of follicular units into the recipient areas is the same in both. In other words, both approaches result in a Follicular Unit Transplant; sometimes referred to as an FUT. This creates a little potential for confusing the terms, or causing one to believe that an FUE is entirely different from an FUT. Actually, FUE only differs by the method of how the donor follicles are obtained.

While the strip method of harvesting remains the more common method, there can be no denying that FUE continues to gain momentum and popularity as the techniques and equipment to best perform the extractions has become increasingly efficient and sophisticated. These advancements in extraction technology have been coupled with a growing body of experience and expertise amongst a relatively small but growing percentage of hair restoration surgeons.

FUE; when performed by an experienced hair restoration surgeon; generates results comparable to conventional strip methods without the risk of creating a linear donor area scar as residual evidence of the transplant. This is the primary benefit that FUE offers. It may be particularly attractive to the person who desires to wear their hair extremely short in the back, or perhaps has already been wearing their hair shaved or cut very short to “conceal” their hair loss. Often these clients want to reserve the option of wearing their hair short now, or in some foreseeable future.

Although modern day closure techniques make donor area scars of lesser concern, it is true that shaving the head, or wearing the hair very short after strip harvesting may reveal the linear scar cosmetically. Although FUE is technically not “scar less” as some would market it, it is true that the small 1mm extraction sites heal quickly with minimal if any visible scar that quickly and easily heals and blends in with even very shortly cut hair.

For others who don’t wear their hair very short (less than ½”), nor foresee wanting to shave or cut their hair very short in the future, FUE may not offer the same practical advantage. Even a modest amount of donor area hair length easily conceals the great majority of scars, and unacceptable scars can easily be corrected or revised. Still, this fact does not preclude FUE from being an acceptable alternative in just about any situation. There are still many clients who prefer FUE for reasons unrelated to concerns over linear scars that might be made visible with short hair. Some simply prefer not to have an incision, or perhaps have a fear or uneasiness about any kind of traditional “surgery”. These fears may have kept them from the enormous benefits that can be obtained by a hair transplant procedure. For them, FUE is a way past this barrier.

Others still, simply desire to have the latest techniques and the most advanced technology at work in making their hair restoration a reality…….nothing wrong with that.

FUE typically involves a degree of greater cost. However, the “value” of a hair restoration procedure is hard to put any price tag on! It’s something that continues to return year after year; and in both real and intangible ways that are related to self-image, confidence, and one’s sense of return to wholeness.

The engineering and manufacturing of specialized FUE equipment has accelerated the popularity of FUE, while at the same time making this otherwise tedious process easier for both patient and surgeon. The manufacturers of FUE devices have even successfully “branded” the names of their devices onto the procedure itself. The great majority of these “brand named” FUE devices are improvements of the control of hand held devices that still require a very experienced and practiced surgeon to manipulate, and most still require substantially greater time to complete a like number of grafts when compared to the strip method.

I have continued during this time to gain FUE experience with the basic “hand engine”; the fundamental and common part of almost all “branded” FUE devices. I have patiently watched to see if FUE would actually become a viable alternative to strip harvesting. I would say it certainly has.

I have also watched closely to see which FUE technology is poised to step out into the open above and beyond the rest. In my opinion, which has been formed after several years of performing FUE, there is a technology that has emerged as the “future-now” of the FUE transplant, and could possibly be poised to revolutionize the hair transplant industry entirely.

The technology I speak of is robotics. Precision robotic FUE has now an achieved technology that has emerged and has been made available to a very select group of hair surgeons worldwide. Presently there are fewer than 35 robotic units in use in the US. To call it “impressive” would be a gross understatement! It’s more like mind-blowing!

The machine is called the ARTAS Robot, and is manufactured by Restoration Robotics; Inc. This is the same company that engineered and developed the Da Vinci Robot that is now used in hospitals across the nation and worldwide to perform minimally invasive cardiovascular procedures such as heart valve repairs. The technology is rapidly spreading into urological procedures and other general surgeries as well.

ARTAS was developed with a clever foresight of how the hair transplantation procedure, and particularly FUE, could be greatly benefited by robotic technology and the precisions laser guidance systems and robotic motions that cannot be reproduced with the human hand. It is much faster, even as it is also more precise in targeting, spacing, and extracting high quality follicular unit grafts for transplantation.

In my estimation, from first hand, real life observation, ARTAS is the future-now of FUE hair restoration. When the future and now meet each other in time, it’s time to make a move!

Hair cloning, also called “Stem Cell Re-genesis”, remains in phase 2 human clinical trials, where it has remained “stuck” for the past several years. Although researchers are staying rather tight lipped on the study, the word on the street is that results are not as promising as had been hoped based upon animal models prior to the human trials. It appears therefore that any commercial application of follicular stem cell technology remains years off. I will report more on the topic as new information is made available through professional channels.

Post-Transplant Care Advice

There are certain vitamins and minerals that not only support hair regrowth, but improve many other aspects to your new hair such as preventing breakage, repair damaged follicles, and even hydrate your scalp. After any hair transplant surgery its best to take all the necessary precautions you can to ensure your new hair is healthy and to your liking. For most people, the approach to vitamin therapy is no different than what might be usually recommended for general health and Vitamin supplementation. The average American diet is fortified by essential vitamins added in food processing, as well as the vitamins normally found in healthy foods themselves.

Because of this, my approach to vitamin therapy for hair health is little different than what I would recommend for general health. A good, broadly inclusive, antioxidant vitamin pill is generally all that is needed. I personally like to supplement with essential fatty acids; such as a combined fish oil and flax seed capsule.


Biotin is a naturally occurring vitamin that is found in a number of common foods. This vitamin helps the body metabolize fatty and amino acids so that they can efficiently be used for energy. It has probably received the most attention as a potentially beneficial supplement for hair health, but again most multi-vitamin supplements contain adequate amounts of Biotin.

One of the early signs of a biotin deficiency in the body is unexplained hair loss and hair breakage, as well as dry skin and brittle nails. People with these signs might benefit from a trial of supplemental Biotin.

Vitamin B-1 is an important vitamin that helps support hair regrowth. Also called thiamine, this water-soluble vitamin improves damaged follicles, hydrates, prevents future breakage, and increases stimulation in blood flow to the area. The average daily dosage recommended is 1.2 mg for men and 1.1 mg for women.

Vitamin C is not only a vitamin, but also an antioxidant that works to strengthen your immune system and protect your scalp. It aids in hair growth, heals follicles, hydrates your scalp and increases blood flow. 1000 mg for adults is what is recommended.

Vitamin E fights to improve your immune system protecting your scalp from infections, diseases, toxins, and viruses that can reduce the rate of hair regrowth. It repairs damaged hair follicles, increases blood flow and strengthens the blood vessels in your scalp. The amount necessary is 15 mg for adults. Minerals:

Iron is an assisting mineral in support of hair regrowth, the aid of red blood cell reproduction, transports oxygen and blood to your scalp which prevents anemia. Anemia can increase your risk of hair loss. The recommended daily dosage of Iron is 8 mg for men and 18 mg for women.

Zinc also aids in boosting your immune system. Zinc encourages heals scalp wounds and accelerates the healing process. Dosage recommended for men is 11 mg and 8 mg for women.

Magnesium and Vitamin D are two other essentials that are sometimes found deficient. Magnesium and Zinc are often combined for convenience as a supplement.

It is important not to overuse vitamins either. There are a number of “hypervitaminosis” conditions that can result from excess use of vitamin and mineral supplements.

In summary, vitamins are important, but most people are not deficient in vitamins assuming they eat a fairly typical and non-fad diet. A good antioxidant spectrum multi-vitamin is fine as a supplement, and essential fatty acids might also be considered. Magnesium, Zinc, and Vitamin D deficiencies can be determined by blood testing if indicated.

For those who may be interested in the FUE method, here are some further details about how it differs from the more common strip method:

The FUE procedure involves the removal of individual follicle units one at time. Because there is no donor area incision line, it offers less immediate post-operative discomfort, faster healing with no sutures, and a quicker return to physical activity than what a strip procedure can offer you.

During the FUE procedure, a specialized “hand engine” tool is used with a small punch instrument, to extract individual follicular unit grafts from a patient’s donor areas. Then the grafts are transplanted into the patient’s thin or balding areas in the usual manner.

Typically the patient‘s hair where the grafts are being removed must be cut very short, so that the physician can see the patient’s scalp and place the extraction bit directly over the units of hair to be extracted.

This procedure is time consuming relative to the conventional strip harvesting method. Because of this, FUE procedures have practical limits of 500 to 1000 follicular unit grafts per session, although there is no need for delays between sessions if more than one is required. The extraction method leaves a small (less than 1.1 mm) round punch hole where each graft has been removed. This tiny wound contracts as it heals making the resulting round scar smaller and less noticeable. While it cannot truly be said that FUE is “ scar-less” surgery, it can be said that the healed areas are largely undetectable to the naked eye once the patient’s hair grows out even a short amount. This allows the patient to continue to wear a short haircut style without risk of any visible linear scar. Healing time varies with each patient, but typically it takes 7 to 10 days until evidence of the surgery fades revealing little or no scarring.

Those contemplating an FUE procedure will want to weigh all factors before choosing this method. While the above advantages are available with FUE, conventional strip harvesting still represents the method of choice for the majority of transplant candidates.

New techniques in donor area management and would closure, have greatly reduced the likelihood of a visible linear scar following a strip harvested procedure. Also, the strip harvesting method requires shaving of only the strip to be removed, and the hair just above the suture line has been taped up before trimming, such that it easily falls down to cover the suture line afterwards. Therefore, there is little if any visible evidence of the surgery even immediately afterwards.

In FUE, in all but very small cases, a larger area must be shaved than what can be covered by the hair above the donor areas where the extractions have been made. Therefore, the small wounds would not be concealed, and the shaved area would be exposed immediately afterwards, unless the case size is quite small. However, head coverings such as caps, hats, or bandannas can be used immediately with either method.

In addition, the post-operative discomfort of strip harvesting is short term, and easily managed. Most importantly, strip harvesting offers the efficiency of being able to move much more hair in a single session than FUE. It is also less costly per graft to have a strip harvested procedure.

Keep in mind that the hair transplantation method is the same in both cases in regards to all other aspects of the procedure, such as hairline design, the making of the recipient area slit openings, and graft placement. Both methods are effective, and both have their place in modern day hair transplantation. We can help you determine which method is best suited to your individual needs.

When considering a hair transplant surgery, always study how much it will cost and determine your budget. These procedures such as the FUE are going to be more costly, but some hair restoration companies do take your budget into consideration. They may also persuade you to have a procedure that may not best fit your needs or your budget, or may promote their use of a brand name, high end, extraction apparatus as marketing tools. Do not be dissuaded by “fear mongers” who play upon a person’s natural anxieties about having “surgery” or an unsightly scar. Come and get the real story about hair restoration, and find out what is truly right for you!
